The project leader is the Medical Department of Collegium Medicum of the Cardinal Stefan Wyszy\u0144ski University (UKSW) in Warsaw.<\/strong><\/p>\n<p>The aim of the project is &#8220;validation of the pharmaceutical service&#8221;. The project &#8220;Closer to the patient&#8221; is a program of three-step intervention of a pharmacist in a pharmacy to reduce the mortality of patients from non-adherence causes of drugs in atrial fibrillation.<\/p>\n<p>The project is carried out by scientists of the Cardinal Stefan Wyszy\u0144ski University together with scientists from the Medical University of Gda\u0144sk and the Collegium Medicum of the Nicolaus Copernicus University in Toru\u0144.
